Guidelines
Getting your Child enrolled
Parents interested in enrolling their children at GROWING STARS CDC must meet with our Center Staff to discuss their child's specific needs and to review program policies.
This can be accomplished by calling and making an appointment with the center director or stopping by the center to talk to the Center Staff. The following items must be completed and returned to the center (along with registration fee and tuition) within 3 days of appointment to guarantee a spot for your child.
Child Care Enrollment
This form must be filled out completely by the parent/guardian, sign it and submit it to GROWING STARS CDC prior to the child's first day of attendance.
Information on this form shall be kept current.
When enrolling a child under two years of age, a completed "Intake for Child Under 2 Years" form must also be filed prior to the child's first day of attendance.
Health History & Emergency Plan
This form must be filled out completely by the parent/guardian, sign it and submit it to GROWING STARS CDC prior to the child's first day of attendance.
Information contained on this form shall be shared with any person caring for this child. The Department of Children and Families recommends that parents/guardians and GROWING STARS CDC staff periodically review and update the information provided on this form.
Child Health Report
Health Examinations are required for children attending GROWING STARS CDC. Instructions on health examine procedures are outlined on the forms.
Day Care Immunization Record
State Law requires all children in Child Care Centers to present evidence of immunization against certain diseases within 30 school days (6 calendar weeks) of admission to the Child Care Center.
Transportation Permissons
The parent/guardian should complete this form for placement in the child's file at GROWING STARS CDC and update the information as needed. GROWING STARS CDC shall maintain the completed form in the child's file for the duration of the child's enrollment. A copy of this form shall be carried in the vehicle when transporting the child.
Alternate Arrival Release Agreement
Complete this form for placement in the child's file when the child will arrive at GROWING STARS CDC from school, home, or other activities, or depart from GROWING STARS CDC to go to school, home, or other activities, and the child will not be accompanied by a parent or other previously authorized person or transported by the Center.
